If you have any question about our fee schedules, feel free to contact our office to request an initial consultation. Also, please take note that depending on the conditions of your actual insurance benefits such as the portion of your deductible already met or your co-pay amount, the out-of-pocket amount will vary.
Our office accepts the following payment methods: All major credit cards (Visa, MasterCard and American Express), debit cards, and checks. Please be aware that you must pay the fee upon the completion of your consultation or treatment session due to our policy.
Please be advised that Dr. Matsuki is not listed as an in-network provider for any insurance panels as of now. Your claims will be submitted by our office to your insurance company for reimbursements if your insurance plan offers out-of-network benefits. Before you receive any treatment, an estimated financial responsibility is calculated and provided, based on specific benefit levels on your active insurance coverage. If you need any assistance, please contact our office any time because we’re always willing to help. If a pre-authorization must be obtained before receiving our service, our office will contact your insurance company.
For whatever reasons, if you need to cancel or reschedule your session, you are required to give us two business days’ (48 business hours’) notice, as a rule. For example, if you need to cancel or reschedule an appointment for Monday, our office must be notified on or before the preceding Friday. Due to our cancellation policy, cancellation of a session within 48 business hours, or failure to cancel and miss a session, will result in paying the appointment fee in full.
